What kind of advice do you need? Please be a bit more specific.
Also, have you considered buying vBulletin or Invision Power Board? Honestly, they are the best forum scripts around.

MySQL Server Name: mysql
MySQL Username: {Database Name - For example, XXXX_Forum}
MySQL Password: {Database Password}
MySQL Database Name: {Database Name - For example, XXXX_Forum. BE SURE YOU ARE INCLUDING THE NUMBERS BEFORE UNDERSCORE!}
MySQL Table Prefix: {Whatever you want here}

* The mysql server is "mysql" do not use "localhost".
* To create a mysql database goto "MySQL databases" in your control panel.
