SMF Setup help Needed

I usually dont deal with dtabases so i ahve no idea what to enter where... and I knwo some poepl will say, yeah, SMF is for noobs... well, I know that but im gonna have SMF up for a while so I can customize my IPBoard to my liking... once thats done, (which will take a while, cuz i pay attention to great detail) so, here is a scren shot of what i need to enter and where i need to enter it... so where does what go?

READ what you posted.
where it says SERVER NAME thats where the server name goes, where it says USERNAME thats where the USERNAME goes, where it say PASSWORD thats where the password goes, where it says DATABASE name thats where the "database" name goes.

Leave the prefix alone if you dont know what it is.

Do I really have to draw arrows on your diagram or can you not copy and paste?

yeah i was going to say servername mysql and the username and DB name both should be 12731_dbone
