PHPBB/ MySQL Help

Hi, on installing phpBB2 it is asking me for a:
Database Server Name/DSN:
Database Name:
Database Password:
Prefix for tables in Database:
I'm new to this what should i put there??
Also why won't it let me upload a .htacess .sh or a file without an exstension, these are in the phpBB folder?

Also one more query can anyone give me FTP settings?

you don't need .htaccess .sh nor extended files in it, want proof, http://forum.arenlorcomics.ulmb.com
Database Server Name/DSN: I believe that will be mysql
Database Name: that's whatever your mysql database name is that you made
Database Password: you set it wehn you created that database (use the mysql link in the cp)
Prefix: well you can leave it blank if it'll let you, you really don't need to have one on here.
And what settings you need for FTP?

Yes those settings worked many thanks. how would i get it to install into mysite.ulmb.com/forum instead of /phpBB? Would i have to rename the main folder from phpBB to forum and then type in forum on the installation page as script path?

I need the:
Hostname or FTP address of web server (do not know this i tried ftp.ulmb.com and did not work)
The folder to store files in: (usually just /)
FTP Login: (Same as my ulmb login i assume)
FTP Password: (Same as my ulmb password i assume)

Yes, just rename the folder and change the script settings to match.

Everything is listed in your control panel here: http://account.ulmb.com/sites/ click view site info on the site you want the ftp settins for.
