How do I get a personalised icon in the address bar - e.g. here you have a vbulliten icon. I've always wondered how to do it...

That's a favicon. Here's a link to a generator that does it for you automatically using an image you specify:

If there's already a favicon.ico file in your site, replace it with your new one.

If you replace an old one, it may take a while for your browser to catch on, idk why.
